Treatment Decisions
Participate actively in decisions about your care, including the right to decline recommended interventions.
Financial Clarity
Know the cost of services and your financial responsibilities before treatment begins.
Confidentiality
Privacy protection for your health information within the limits established by law.
Record Access
Request and receive copies of your clinical records, and request amendments when appropriate.
Insurance Choice
Decide whether to use insurance benefits or pay privately for services.
Voice Concerns
File complaints or appeals regarding your care without penalty or discrimination.
End Treatment
Discontinue services at any time.
Complete information about your rights, confidentiality protections, treatment processes, and financial policies is provided in your informed consent and practice policy documents during the onboarding process.
